Life Insurance and AD&D

Employment

Toray provides employee life insurance equal to two times your salary effective the first of the month following 90 days of employment. You can "buy down" your coverage and receive the difference in cash or credits; or "buy up" and pay the difference with pre-tax dollars. The following are your life insurance coverage options:

• $5,000
• One Times Salary & AD&D
• Two Times Salary & AD&D
• One Times Salary Supplemental & AD&D

Long-Term Disability Insurance

Toray provides long-term disability (LTD) insurance coverage in the amount of 60% of monthly salary after a 180 day waiting period. The LTD benefit is effective the first day of the month following 90 days of employment. Your LTD options are as follows:

• 60% of monthly salary after 180 days
• 40% of monthly salary after 210 days

Dependent Life insurance

Dependent life insurance is an optional benefit that you can elect to cover your spouse and dependents. Dependent life insurance coverage is effective the first day of the month following 90 days of employment. The following are your options:

• $5,000 spouse benefit
• $15,000 spouse benefit
• $25,000 spouse benefit
(Dependent children are covered up to $2,500 each)
